APPOINTMENTS

Please call to schedule an appointment. Initial evaluation appointment includes an evaluation, diagnosis and treatment. This session lasts for an hour. The therapist will discuss your diagnosis, perform a treatment and create a customized treatment plan for you. Please wear loose clothing or bring clothes to change into, especially if you are having your knee treated.

 Follow up appointments will consist of treatment and re-assessment of your condition.  This appointment will be a 30-60 minute session dependent on your choice of appointment length. These sessions will be customized to your needs as a patient.

Self pay rates are offered. Payment is due at time of service. I do not no accept insurance.

Cash, check and credit cards are accepted forms of payment. 

Please give 24 hours notice if you are unable to keep your appointment.  Any cancelled visit with less than 24 hours notice will be subjected to a $50.00 cancellation fee.
